Geographic Atrophy Screening Program - GATEway

Cureos is proud to launch the first digital referral system (GATEway) in partnership with Consultmed, dedicated to screening patients with Geographic Atrophy.

GATEway is an Advice & Guidance service to support optometrists and Healthcare practitioners wishing to seek appropriate treatments for their patients with geographic atrophy (GA).

This is not a referral, but rather a request for treatment recommendations which can reduce the need for unnecessary specialist consultations. For all other eye conditions, please use alternate referral pathways. This form is currently for Geographic Atrophy. GATEway uses the Consultmed digital referral system, ensuring smooth communication and secure data transfer. Referring practitioners will receive a response within 2-3 business days.

Submit an Advice and Guidance request

If you do not wish to create an account, you can also submit a request by visiting the external portal

Complete the clinical sections

Ensure that you complete the clinical sections relating to BCVA, IOP, Relevant ocular and family history.

Upload relevant images

Upload relevant images (e.g. OCT through the fovea, FAF image (if available). Photos taken on the phone are accepted.

Submit the request

Submit the request and keep an eye out on your emails for a response within 2-3 business days.
